Legal Agreements for Internet Therapeutic Services:
1. I certify that I am an adult, meaning 18 years old. If you are under 18, you cannot enter this therapeutic service.
2. I understand that this a professional setting and I will not use this therapeutic session as a sexual “chat room” or use it for any other inappropriate sexual intent. If your intension is not to receive professional therapeutic services, please do not proceed with the session.
3. I understand that if I violate any above agreements or boundaries, or become sexually inappropriate, the therapist has the right to terminate the session. There will be no refund of money if a session is terminated by the therapist.
4. I understand that each session will remain confidential and anonymous and will not be given to anyone to see or review except by the therapist. However, each session will still be recorded, without the name of the client, for professional and legal requirements and stored in a safe and secured place. In case of a legal dispute, each recorded session maybe reviewed by management of the corporation or attorneys working for the corporation.
5. I understand the cost for each session is $80 per 50 minute session and must be paid prior to each session.
6. If there is a technical glitch in the system whereas you cannot connect to the therapist after you have paid for the session, your money will be refunded in full.
7. To receive maximum benefit, you should attend many therapeutic sessions. While internet treatment is a good start to help people begin their treatment, personal one-on-one sessions with a therapist is more effective and desirable. There is no such thing as a quick fix in sexual addiction!
8. Binding Arbitration Agreement, Non-legal Agreement. This agreement requires that you submit all future claims to arbitration instead of having the claim heard in court by a judge or jury. This agreement is included to minimize the cost of any disputes that may arise from your contact with the Counseling Center for Spiritual and Moral Development, LLC.
9. The Counseling Center for Spiritual and Moral Development, LLC will not accept insurance coverage for internet therapy.
10. By checking the box, I certify that I have read all the agreements in this document and accept its terms. If I do not agree with the document, please do not proceed.
